- Esomeprazole
Indications
Esomeprazole is indicated for the relief of chronic heartburn and other symptoms associated with gastroesophageal reflux disease (GERD).
Pharmacology
Esomeprazole is a proton pump inhibitor (PPI) that suppresses gastric acid secretion by specifically inhibiting the H+/K+-ATPase enzyme in the stomach's parietal cells. As the S-isomer of omeprazole, Esomeprazole provides better acid control compared to racemic proton pump inhibitors.
Dosage
- Healing of Erosive Esophagitis: 20 mg or 40 mg once daily for 4-8 weeks. Most patients heal within this time. For those who do not, an additional 4-8 weeks of treatment may be considered.
- Maintenance of Healing of Erosive Esophagitis: 20 mg once daily (clinical studies have not extended beyond 6 months).
- Symptomatic GERD: 20 mg once daily for 4 weeks. If symptoms persist after 4 weeks, treatment may be extended for another 4 weeks.
- Helicobacter Pylori Eradication (Triple Therapy): Esomeprazole 40 mg once daily for 10 days, alongside Amoxicillin 1000 mg twice daily and Clarithromycin 500 mg twice daily for 10 days.
- Zollinger-Ellison Syndrome: 20-80 mg once daily. Dosage should be individualized, and treatment should continue as long as clinically necessary.
- Acid-Related Dyspepsia: 20-40 mg once daily for 2-4 weeks, depending on response.
- Duodenal Ulcer: 20 mg once daily for 2-4 weeks.
- Gastric Ulcer: 20-40 mg once daily for 4-8 weeks.
- Injection: 40 mg Esomeprazole once daily via intravenous injection (no less than 3 minutes) or intravenous infusion (10 to 30 minutes). Esomeprazole IV should not be mixed with other medications in the same IV site. Treatment with Esomeprazole IV should be discontinued once the patient can resume oral therapy.
Administration
- Tablet or Capsule: Swallow whole and take one hour before a meal.
- Delayed-Release Oral Suspension: Mix the entire contents of the packet with 15 ml of water, stir well, and let sit for 2-3 minutes to thicken. Stir again and drink within 30 minutes. If any medicine remains, add more water, stir, and drink immediately. For nasogastric or gastric tube administration, use 15 ml of water in a syringe, shake well, let sit for 2-3 minutes, then shake and inject through the tube within 30 minutes.
- IV Injection: Administer Esomeprazole slowly through IV over at least 3 minutes. Reconstitute the injection with 5 ml of water for injection (WFI) and use within 12 hours if stored at room temperature (up to 30°C). Do not refrigerate, and do not use if particulate matter is visible.
Interactions
Esomeprazole is metabolized in the liver primarily by CYP2C19 and CYP3A4 enzymes. Studies show that Esomeprazole does not inhibit CYP enzymes such as 1A2, 2A6, 2C9, 2D6, 2E1, and 3A4, meaning it is unlikely to cause clinically relevant interactions with drugs metabolized by these enzymes. Studies have shown no significant interactions between Esomeprazole and phenytoin, warfarin, quinidine, clarithromycin, or amoxicillin.
Contraindications
Esomeprazole is contraindicated in patients with known hypersensitivity to any of its components.
Side Effects
The most commonly reported side effects of Esomeprazole include headache, diarrhea, nausea, flatulence, abdominal pain, constipation, and dry mouth. Long-term maintenance therapy (up to 12 months) shows no differences in the types of side effects compared to short-term treatment.
Pregnancy & Lactation
There are no adequate, well-controlled studies on the use of Esomeprazole in pregnant women. Animal studies have shown no teratogenic effects. The excretion of Esomeprazole in human breast milk has not been studied, so breastfeeding should be discontinued if Esomeprazole use is necessary.
