Presentation
- Carvista 6.25: Each film-coated tablet contains 6.25 mg of Carvedilol.
- Carvista 12.5: Each film-coated tablet contains 12.5 mg of Carvedilol.
- Carvista 25: Each film-coated tablet contains 25 mg of Carvedilol.
Description
Carvista (Carvedilol) is a cardiovascular medication primarily acting as a non-selective antagonist of β-adrenergic receptors while also demonstrating significant α-adrenergic antagonistic properties. Additionally, it has antiproliferative effects and acts as a scavenger for reactive oxygen species. It is indicated for the treatment of hypertension, angina pectoris, and congestive heart failure.
Indications
Carvista (Carvedilol) is indicated for:
- Treatment of mild to moderate heart failure (NYHA class II or III) of ischemic or cardiomyopathic origin, in conjunction with digitalis, diuretics, and ACE inhibitors, to help reduce disease progression as evidenced by cardiovascular death, hospitalization, or the need for adjustments in other heart failure medications.
- Use in patients who cannot tolerate ACE inhibitors.
- Use in patients not receiving digitalis, hydralazine, or nitrate therapy.
Dosage & Administration
For Heart Failure:
- Start with 3.125 mg twice daily with food.
- Gradually increase the dose at intervals of at least 2 weeks to 6.25 mg twice daily, then to 12.5 mg twice daily, and up to 25 mg twice daily. The maximum tolerated dose is 25 mg twice daily for patients under 85 kg and 50 mg twice daily for those over 85 kg.
For Hypertension:
- Begin with 12.5 mg once daily, increasing to a usual dose of 25 mg after 2 days. Further increases can occur every 2 weeks as needed, up to a maximum of 50 mg daily, given in a single or divided dose. For elderly patients, an initial dose of 12.5 mg daily may suffice.
For Angina Pectoris:
- Start with 12.5 mg twice daily for the first 2 days, then increase to 25 mg twice daily. In elderly patients, the maximum daily dose should not exceed 50 mg in divided doses.
Side Effects
Common side effects may include:
- Postural hypotension
- Dizziness
- Headache
- Fatigue
- Gastrointestinal disturbances
- Bradycardia
- Reduced peripheral circulation
- Peripheral edema
- Dry mouth and eyes, eye irritation, or vision disturbances
- Impotence
- Urination issues
- Flu-like symptoms
- Rarely, angina, AV block, worsening of intermittent claudication or Raynaud's phenomenon, allergic skin reactions, exacerbation of psoriasis, nasal congestion, wheezing, mood changes, sleep disturbances, paresthesia, heart failure, changes in liver enzymes, thrombocytopenia, and leukopenia.
Precautions
- Use with caution in patients with hepatic impairment.
- Monitor clinical status for 2-3 hours after initiating therapy or increasing doses. Ensure renal function and heart failure status are stable before dose adjustments.
Use in Pregnancy & Lactation
- Pregnancy: Carvedilol should not be used during pregnancy due to insufficient studies, though animal studies indicate it crosses the placental barrier.
- Lactation: Carvedilol is excreted in breast milk; therefore, its use during breastfeeding is not recommended.
- Children: Safety and efficacy in children have not been established.
- Neonates: There is no available data on the safety and efficacy of Carvedilol in neonates.
Drug Interactions
- Digoxin: Co-administration can lead to significantly increased digoxin levels; therefore, close monitoring is advised when starting, adjusting, or discontinuing Carvedilol.
- Rifampin: This antibiotic may decrease Carvedilol levels by 60%.
- Warfarin: Carvedilol does not significantly alter the plasma protein binding of warfarin.
- Clonidine: Beta-receptor antagonists may potentiate the pressor response following sudden withdrawal of clonidine, although Carvedilol's alpha-blocking action may mitigate this effect.
