Camlodin Plus Tablet 5mg+50mg
Indications
This medication is indicated for:
- Patients with essential hypertension
- Patients with both angina pectoris and hypertension
- Post-myocardial infarction (MI) patients
- Patients with refractory angina pectoris when nitrate therapy has failed
Pharmacology
Amtinol-plus is a fixed-dose combination of Amlodipine and Atenolol:

Amlodipine: This is a dihydropyridine calcium antagonist that inhibits the influx of calcium ions into vascular smooth muscle and cardiac muscle, primarily affecting vascular smooth muscle. Amlodipine acts as a peripheral vasodilator, reducing peripheral vascular resistance and lowering blood pressure. It decreases coronary vasoreactivity and lowers cardiac demand by reducing afterload.

Atenolol: A cardioselective beta-blocker, Atenolol reduces blood pressure by lowering cardiac output and decreasing plasma renin activity and sympathetic outflow from the central nervous system (CNS). It also reduces myocardial oxygen demand through its negative inotropic and negative chronotropic effects.
Dosage & Administration
The recommended dosage is one Amlodipine and Atenolol 5/25 mg tablet taken once daily. If necessary, the dosage may be increased to two 5/25 mg tablets daily, based on physician advice. Dosage should be individualized.

Interactions

Disopyramide: Atenolol reduces the clearance of disopyramide by 20%, which may result in additive negative inotropic effects.

Ampicillin: Doses of 1 gram or more may reduce Atenolol levels.

Oral Antidiabetics and Insulin: Beta-blockers, including Atenolol, can decrease tissue sensitivity to insulin and inhibit its secretion, though Atenolol has a lower potential for these effects.
Contraindications
This combination should not be used in patients with:
- Hypersensitivity to either component
- Sinus bradycardia
- Second- or higher-degree heart block
- Cardiogenic shock
- Hypotension
- Congestive heart failure
- Poor left ventricular function
Side Effects
The combination of Amlodipine and Atenolol is generally well tolerated. Common side effects include:
- Fatigue
- Headache
- Edema
- Nausea
- Drowsiness
- Anxiety
- Depression
Pregnancy & Lactation
The combination should be used during pregnancy only if the expected benefits outweigh the potential risks to the fetus. It is not recommended for nursing mothers. If necessary, breastfeeding should be discontinued.
Precautions & Warnings

Bronchospasm: Use with caution in patients with airway obstruction.

Renal Impairment: The combination can be used in patients with renal impairment, but caution is advised if creatinine clearance is less than 30 ml/min due to potential reduced excretion of Atenolol.

Hepatic Impairment: Use with caution in patients with severe liver damage, as Amlodipine's elimination half-life may be prolonged.

Drug Withdrawal: Patients should be cautioned against sudden discontinuation, as coronary heart disease may be present without symptoms. Any discontinuation should be gradual and monitored.
Overdose Effects
Although not well documented, overdose may lead to hypotension and, less frequently, congestive heart failure. Unabsorbed drugs may be removed through gastric lavage or activated charcoal. Symptomatic treatment is recommended.
